Why this firm exists.

Austin Healthcare Advisory was founded on a frustration familiar to anyone who has watched an independent practice struggle: the help available to physicians is mostly advice — and advice was never the missing piece.

The Firm

From advisor that talks to operator that does.

Independent practices in distress don’t lack opinions. They get assessments, benchmark reports, strategic recommendations — each one plausible, none of them executed. The consultants present, invoice, and disengage. The practice returns to Monday morning with the same overloaded manager, the same stalled partnership, the same declining cash.

John Austin built AHA from inside practice operations — close enough to the front desk, the billing queue, and the physician-owners’ meeting to know that the gap was never analysis. It was execution: someone with the authority to change how the practice runs, the judgment to do it without disrupting clinical work, and the accountability to stay until the change holds.

So the firm is built around that missing piece. AHA takes the operator’s seat — temporary, defined in writing, accountable for outcomes — and works until the practice doesn’t need it. The people on the engagement are the people doing the work.

John Austin

Founder & Principal

John founded Austin Healthcare Advisory after more than twenty years across healthcare operations, financial strategy, and enterprise systems.

He has operated in COO capacities inside independent practices and led financial and operational strategy within the Mass General Brigham system, supporting over $1B in patient service revenue — capital planning, operational performance, and the financial and reporting infrastructure that complex clinical operations run on.

Earlier in his career, John worked in private credit and sponsor finance, including roles at J.P. Morgan and Antares Capital — the foundation in financial analysis, capital structures, and risk management that the firm’s cash-first doctrine still reflects.

His approach is grounded in execution: working inside practices to restore financial visibility, align operations, and build systems that support long-term stability and growth.

Vanessa McLellan

Client Operations

Vanessa leads client coordination and administrative operations, supporting the day-to-day execution of every engagement. She works closely with practice leadership and staff to manage communication, align expectations, and keep operational initiatives organized and on track — overseeing administrative workflows, coordinating recruiting and staffing efforts, and supporting consistent execution across the engagement.

She brings a calm, structured approach to complex environments and handles evolving priorities with discretion and responsiveness. Details get handled, communication stays clear, and the engagement keeps moving — even in challenging or rapidly changing situations.
